Any chance of a registered nurse and licensed long term care administrator (US licensure) working in or around La Paz - say as an English speaking nurse, experienced in geriatric nursing and administration for an Assisted Living? or something related?

First step: sign up for Spanish Immersion Classes at Se Habla La Paz.
Ther owner is a former hospital administrator form AZ. She started her school to help hospital workers learn Spanish by attending her first class school, staying with Mexican families and working side by side with doctors. When they return to the US they have a good command of Spanish, and a new attitude.
This would be a place to start.
